If you are transferring from another physician, our office can assist with obtaining your previous medical records. A Medical Records Release Authorization form is included in the New Patient Packet for your convenience.
Medicare & Insurance Information
Family Doctors of Pasadena accepts Medicare and Humana Medicare Advantage plans. If you are enrolled in a Medicare HMO plan, your primary care physician plays an important role in coordinating specialty care, managing referrals, and helping ensure continuity of care throughout your healthcare journey.

Advance Directives
Advance directives help communicate your healthcare wishes should you become unable to make medical decisions for yourself. Florida recognizes both Living Wills and Healthcare Surrogate Designations. Advance Directive forms and educational materials are available within our New Patient Packet.
